This Privacy Policy describes the types of personal information we collect about you through our Services, how we may use the information, with whom we share it, and the choices available regarding our use of the information.

    • Categories of personal information that we disclosed about you for a business purpose and the categories of persons to whom it was disclosed for a business purpose
  5. Right to Opt Out of Sale or Sharing of Personal Information: You have the right to direct CreatorHub, if we sell or share your personal information to third parties, not to sell or share your personal information.
  6. Right to Limit Use and Disclosure of Sensitive Personal Information: You have the right to direct CreatorHub to limit the use of your sensitive personal information to the use which is necessary to perform the Services.
  7. Right of No Retaliation Following Opt Out or Exercise of Other Rights: We may not discriminate against you if you exercise any of your rights as described in this Section VIII.
  8. Right to Data Portability: You have a right to obtain the personal information in a portable and, to the extent technically feasible, readily usable format that allows the consumer to transmit the information to another entity without hindrance. You may exercise this right no more than two times per calendar year.

IX. NOTICE OF RIGHT TO OPT-OUT OF SALE/SHARING

We share your personal information to provide you with cross-context behavioral advertising about our Services. You can submit a request to opt-out of sale/sharing of your personal information by emailing us at support@physicianschoice.com. If you exercise your right to opt-out of sale/sharing of your personal information, we will delete your profile from the Services.

X. METHODS FOR EXERCISING DATA SUBJECT RIGHTS

You can submit a request to exercise one or more of your data subject rights stated above using the following methods:

After we receive your request, we will determine if the request is a verifiable customer request before acting on the request. You must provide enough information for us to verify that it is the proper person making the request; if we cannot complete the verification, the request will not be processed. We have 45 days in which to take action on your request. If we run into issues in honoring your request, we will notify you within those 45 days. The original 45-day period may be extended by 45 days if reasonably necessary. We will inform you, without delay, of the reasons, if we decide not to take action on your request. We will also inform you if you have rights to appeal our decision.

If you use an authorized agent to submit a request and the authorized agent does not provide a proper power of attorney, we may require you to either (1) verify your identity directly with us, or (2) directly confirm with us that you provided the authorized agent permission to submit the request.

XI. OPT-OUT PREFERENCE SIGNAL

If your browser supports an opt-out preference signal, you may opt out of the sharing or sale of your personal information by properly configuring the opt-out preference signal. If you subsequently delete cookies, change your browser settings, or use a different browser, you will need to turn the opt-out signal on again.

XII. REQUEST TO OPT-IN AFTER OPTING OUT

Some functionality within our Services may require us to sell or share your data. For example, if you have opted out of us selling or sharing your information and now want to opt-in to the selling or sharing of your information in order to access such functionality within our Services and you are opted out we will inform you that the functionality requires the sale or sharing of your personal information and provide instructions on how you can provide consent to opt-in to the sale or sharing of your personal information.

XV. CHILDREN'S PRIVACY AND AGE-RELATED INFORMATION COLLECTION POLICIES

If we have actual knowledge that we have collected personal information from someone under the age of 18 we will delete that personal information. If you are a parent or guardian and believe your child has provided us with personal information, please contact us. If we become aware that we have collected personal information from children without verification of parental consent, we take steps to remove that information from our servers.

We do not sell the personal information of consumers we know are less than 16 years of age, unless we receive affirmative authorization from either the consumer who is between 13 and 16 years of age, or the parent or guardian of a consumer less than 13 years of age. The parent or the child who has opted into personal information sales or sharing may opt out of future sales at any time. If a parent, guardian or child has provided affirmative authorization, they may opt out of future processing at any time by contacting us at support@physicianschoice.com.

XVI. LINKS TO THIRD PARTY WEBSITES

Our Services may contain links to third party websites, services, or resources that are not covered under this Privacy Policy and our Terms of Service. Upon visiting or using those third-party websites, services, or resources, we recommend that you review their privacy policy, terms and conditions, and terms of use before providing your personal information.

XVII. TRANSFER OF CORPORATE ASSETS

If CreatorHub is acquired by or merged with a third-party entity, we reserve the right to transfer or assign the personal information you provided to us as part of such merger, acquisition, sale, or other change of control to the new entity. In the event of bankruptcy, insolvency, reorganization, receivership, or assignment for the benefit of creditors, or the application of laws or equitable principles affecting creditor's rights generally, we may not be able to control how your personal information is processed. If such a situation arises, we would request that all applicable privacy laws and protection standards at least as stringent as ours be applied to your personal information originally in our possession. In the event of a transfer of corporate assets, the information regarding the transfer will be posted on our Services.

  • Limit the Information that You Provide: If you have an account with us, you can choose not to provide the optional account information, profile information, and transaction and billing information. Please keep in mind that if you do not provide this information, certain features of our Services may not be accessible.
  • Choose Not to Use Certain Features: You may choose not to use certain features within our Services where the use of such features results in the processing of information you submit through the feature by a third party. For example, use of our chatbot feature results in the sharing of information with our chatbot service provider. If you do not wish to disclose information with the third party such features, please do not use such features.
  • Opt-Out of Electronic Communications: You may opt out of receiving promotional messages from us. Just follow the instructions in those messages. If you opt out of promotional messages, we may still send you other messages, like those about your account and legal notices.
  • Set Your Browser to Reject Cookies: Please see Information Collected Automatically above for how to set your browser to remove or reject browser cookies before using our Services, with the drawback that certain features of the Services may not function properly without the aid of cookies.
  • Delete Your Account: If you no longer want to use our Services, you can delete your account by sending your request to support@physicianschoice.com. Please keep in mind that we may continue to retain your information after closing your account, as described in How Long We Keep Personal Information above, for example, when that information is reasonably needed to comply with (or demonstrate our compliance with) legal obligations such as law enforcement requests, or reasonably needed for our legitimate business interests.

We reserve the right to update this Privacy Policy to accurately disclose changes as necessitated by applicable laws and other events. If those changes are material in nature, we will notify you by a notice placed on our public facing Website or by electronic mail.
